Health and Wellness Dentistry ("we," "us," "our") respects your privacy. This Privacy Policy explains what information we collect when you request our free whitening guide or interact with our marketing materials, how we use it, and the choices you have. Please read it carefully.

This policy covers information collected through our marketing landing pages, including our free patient education guide opt-in form. It does not cover protected health information (PHI) collected during clinical care at our offices, which is governed separately by our HIPAA Notice of Privacy Practices, available at any office.

• Email address
• Technical metadata: IP address, browser user-agent, page URL, referring URL, marketing campaign parameters (UTM tags), the closest office tagged in the link you arrived from (if applicable), submission timestamp, browser language and time zone

We do not collect phone numbers, medical history, diagnoses, insurance information, or any protected health information through this form.

3. How we use your information

• Deliver the requested guide. We email the PDF to the address you provided.
• Send marketing email communications. By submitting the form, you agree to receive periodic follow-up emails covering dental health tips, news, and special offers. Every email has a one-click unsubscribe link, and we honor opt-outs within 10 business days as required by the CAN-SPAM Act.
• Improve our outreach. Aggregate, de-identified analytics on which campaigns bring patients to us.
• Comply with law. Honor opt-out requests, respond to lawful requests from authorities.

4. Legal bases & compliance

We process your information based on:

• Implied consent at form submission (clickwrap) for marketing email communications, as permitted by the federal CAN-SPAM Act. The exact clickwrap text shown at submission is recorded with your IP address and timestamp.
• Legitimate interest in delivering the educational material you requested.
• Legal obligation to retain certain communications and audit records.

Florida advertising compliance: this policy and our marketing materials operate under the supervision of Dr. Naved Fatmi, DMD, the licensee responsible for our advertising contents (Florida Statute §456.072).

Access, correct, or delete

We maintain reasonable administrative, technical, and physical safeguards designed to protect your information from loss, misuse, or unauthorized access. No internet transmission or electronic storage is 100% secure; we cannot guarantee absolute security.

We retain your contact information until you opt out or request deletion, plus a reasonable retention period for our records (typically up to seven years for compliance and audit purposes).

9. Children

Our marketing materials are not directed to children under 13, and we do not knowingly collect information from children under 13. If you believe we have, contact us and we will promptly delete it.
